I've just started using a mac so sorry if my information is vague.
in voiceOver preferences in El Capitan there is a feature for voice ducking.
This is on by default and will cause VO volume to change.
If you untick the button this should stop it from changing.
It did for me once I upgraded the machine to the latest version.

Yes, I can confirm the same happens on my macbook pro too. I've only just
noticed it because I normally use my mac with headphones and when the
headphones are plugged in, this change does not happen. So I was very
surprised when one day I disconnected my headphones and i noticed this strange
behaviour.

On a MacBook Pro I have the exact same issues.
It’s pretty annoying, will email Apple Accessibility as well, because Voice
Over has some more bugs. Like using high pitched voice, while this is disabled
also.
