Hi there.
When this scan gear dialog comes up, do the following:
Adjust the settings you want to adjust from that dialog, if i remember 
correctly, there are settings to adjust brightness and so on and maybe they 
don't need adjusting at all but it doesn't hurt to learn the dialog.
Find the text that says "perform scan". When there, if your mouse isn't 
following the vo cursor, route the mouse to the vo cursor by hitting vo-cmd-f5 
then grab the physical mouse and move it slowly backwards (i think of it as 
down) until VO says "enter window" then click the mouse button and the scanner 
will start. This approach is necessary because VO doesn't as far as i know 
support moving the mouse in small steps like one or two pixels.
A note: This approach that i described assumes that you have changed the 
parameter in the voiceover settings>navigation, i think it is, that says 
"Notify when mouse enters a window" or something similar. I only have Swedish 
vo so i don't know the correct wording of that menu choise.
