I have seen that happen before.  My guess is that it has something to do with 
how that particular app is coded.  I’m not familiar with Voiceover’s internals, 
but I imagine it works like other screen readers in that it stores the bitmap 
of the item you are labeling.  If for some reason the app does not construct 
the button in the same way every time, VO can’t identify it.  The state of the 
control, i.e. selected or not) may also affect the ability of VO to detect it 
and you may just need to explore a bit more to figure out what’s happening.  I 
forget the app where I ran into this now but I finally gave up on it because 
nothing I did made the labels stick.  Unfortunately the best answer may be to 
find another app that works better with VO, but it’s a good idea to get in 
touch with the developer if you can and see if they will fix it.  If they 
properly label the buttons in the first place then you don’t have to mess with 
it.
