Just to add to this, I've also seen this.  It only happened once, but somehow 
the phone number for our vet was added as a second phone number to an entry I 
have for a local pizza place.  I noticed it on my iPhone, did not check whether 
that carried over to the Mac as well.  I went into Contacts on my Phone and 
corrected the entry, and have not seen the problem since.
