At least for the latter part of your question, how are you launching iTunes? If I recall correctly, holding down option while launching tells iTunes to prompt for the library location so if you are using some VO command and keeping the key down that might be the cause. The non-VO method such as doing command-o for open on the application doesn't have this snag. Hope this helps.
