Hi,

This could be accomplished in Preview.  I haven't tested it thoroughly, but 
it's more easily done from a sighted prospective.  Once the pdf is open in 
Preview, go up to the View menu and choose ThumbNails.  This will display an 
area with a thumbnail view of each page.  If you VO-space on that page, then it 
would maximize the specific page.  So, what I'm suggesting is that you make 20 
copies of the full pdf while in the Finder using cmd-d to duplicate each copy, 
and naming them Chapter 1 through 20 accordingly.  Then, when you open the one 
named Chapter one, press on any page in the thumbnail view that is not part of 
Chapter 1, then press your Delete key.  Repeat this process until only pages 
that are part of Chapter 1 are present and save the modified pdf.  This sounds 
tedious, but it does the job.  Sighted users are able to select multiple 
thumbnails and delete them on mass, but I'm not sure how easy that is from a VO 
prospective.  May be possible, but I haven't played with that aspect much yet.
