Jonathan, Your method works, I used to do it that way as well before I figured this one out. If you think about it, it's logical. There's quite a number of Contextual menu items available, VO just doesn't always properly access them all. Haven't quite figured out why this is so, but it is. You'll find that this method works in the table of titles in iBooks as well. You can navigate to a title, press VO-shift-m and nothing happens, whereas if you press ctrl and click the mouse/trackpad there'll be a Contextual menu.
