OK.  I can confirm what you are explaining.  As long as you allow FaceTime to 
remain as the app with primary focus, then the buttons with the "End Call" and 
such remain visible and accessible.  As soon as you go out of FaceTime and try 
to return to the app with cmd-tab or some other method, those buttons are gone 
and you're unable to manipulate the current call.  What is happening here is 
that the dialog that you originally see is not actually part of FaceTime, but a 
notification and thus part of the Notification Centre.  Use the VoiceOver 
command VO-n to check for notifications, then just press return, and you should 
be brought back to that dialog with the End Call and Mute options.  If all you 
wish to do is end the call, just press cmd-q while in FaceTime and the call 
will be terminated.  That's less keystrokes than the first method.  Also, note 
that if a second call were to come in while you're on the first, Notification 
Centre would again gain focus and you'd be presented with the "Hold & Accept" 
or "Decline" options, otherwise, in the case where you just have a single call, 
the methods I outlined first should do the trick for you.
