Here is Anne Robertson's description of how to do this. Takes some getting used to, but it works once you get it sorted out. When reading through the document, VoiceOver should read both the deleted text and the inserted text. Command-Option-k should put you in the popover where you should hear what has been deleted and where you can accept or reject the change. Pressing VO-j twice should put you back in the body of the document at the point where you left it.
Command-Shift-K will allow you to enter a new comment.
