Sometimes, it will read as "FaceTime Video" or "FaceTime Audio"
depending on the type of call made or received. So, if it is a regular
phone call, it will read as "primary".

I believe that the label is supposed to be there, it just is being
mishandled by some phones.
So, in my case, it reads 780-123-4567, Incoming, Primary, 1 Call, Fort
McMurray, November 4, at 12:24 pm. Of course, some of those are
subject to change such as Incoming can be Outgoing and the number of
calls can change along with the date and time obviously.
