I don't think that VO users can customize toolbars. My memory is that the
pallets have icons that then need to be dragged into the toolbar of the main
window, however VoiceOver does not allow the VO cursor to move away from the
dialog box while it is open. It might be possible to script something to do
this, but really this should be an item for Apple to consider. I usually
hide/ignore toolbars so I have not really thought about this much, but it could
certainly add some efficiency especially if one creates a hotspot for the
toolbar.
