I'm not sure I'm understanding this correctly but I'll give an answer that I 
hope might help.

 Your apple watch is paired with one iPhone and therefore picks up phone calls 
associated with that phone when a call is received.

 If you have a second phone then you should be able to call it from your 
primary phone and associated apple watch,

If your apple watch is a series 3 or 4  with LTE then you should be able to 
make phone calls to any other device that is if I understand how the LTE watchs 
work.

Hope that makes sense and helps with what your asking.

I haven’t got an Apple Watch and I am no expert by a long shot. But I am 
guessing that is because your Apple Watch is paired with your iPhone, but your 
other devices like your extra phone are not.
