I’ve just tried this on Bluetooth and wifi icons in menu extra but the way I’ve 
accomplished it is:
1. vo+m twice to go to menus extra
2. move to the icon you want, i.e. bluetooth
3. Route mouse cursor to vo cursor with vo+command+f5 and confirm the routing 
with vo+f5
4. disable trackpad commander with vo+two finger rotation left
5. hold down option key and click the mouse pad manually.
