Hello, 

Any voiceOver key can be assigned to any NumPad button with or with out a 
modifier. Certainly the default is to navigate a little like JAWS does with 
smart navigation on, but if you prefer shift 5 tone previous sentence then just 
do it. 
Open up VO-utility and find the Num Pad commander  section of the commanders 
tab. There you will find a table. The last column in the table is a pull down 
where you can select the command you want. If you need functionality beyond the 
list of commands available in vo-h-h then AppleScript or JavaScript for 
Applications can be used for customization. BeJust before the table,  is a pull 
down where you can select no modifier, command, option, control, shift or num 
pad 0 as a modification key.  
If you need different commands for different applications, then you will first 
need to create an activity for that application.
