Hi,

Have you considered the Keyboard commander in the VO Utility.  For example, you 
could do the following:

1.  Open the Vo Utility with VO-f8
2.  Press cmd-8 to bring focus to the Commanders category
3.  VO-right to the Keyboard tab and VO-space on it.
4.  VO-right to the table and interact with it.

Navigate through the list to get and idea of the currently used letters.  Also 
note that, by default, the Keyboard Commander uses the right option key.  So, 
by holding down the right option key and any of those letter listed, you'll get 
the corresponding command.  We can also add our own here.  So, just for fun, I 
tried it and it worked great.  Some books may have issues, but most should be 
fine.  Thus, let's create a few.

5.  Stop interacting with the table and VO-right to the Add button then 
VO-space to press it.

VO will automatically jump back inside the table ready for you to enter a 
letter and associated command.

6.  Type the letter "j", then VO-right to the menu, down to Text, then right 
and down to Previous Sentence.

YOu've now set it so that you can press the right option key and the letter j 
to read the previous sentence.  I'd follow the same steps in #5 and #6 to set 
"k" for current sentence, and "l" for next sentence.  Similarly, you can set a 
group of letters, such as "u, i and o" for previous, current and next 
paragraph.  The letter "u" is already assigned to the "unread Mail count, but I 
never use that one and therefore removed it before creating the paragraph 
commanders.

I'm wondering if this would help your situation some.  As Jonathan mentioned, 
you could also set TrackPad commander gestures to some of these as well if the 
Keyboard commander is still not working well for you.

solution is to use a “chip clip“ to firmly hold these keys in place which

hand nerves. I imagine that a clipboard would also work as long as the jaw of 
the clip was at least as wide as those four keys, but I wonder if the intense 
pressure of a metal clipboard might not damage the keyboard itself. As it is, 
the plastic chip clip has enough force to hold the keys down but I don’t think 
enough to damage the keyboard.
by the way, if anybody doesn’t know what I mean by a “chip clip“, it’s 
basically a plastic wide clip used to reseal a bag of potato chips after it’s 
been opened. And they can be found at any grocery store. Anyway, that’s my 
solution for a problem that was threatening to keep me from being able to read 
in the Books app or on any app that requires use of a keyboard. I know there’s 
another fellow on this list with carpal tunnel and maybe he can get some ideas
