Agree with Tim’s assessment. Audio routing has become, and is continuing to be 
very complicated. We have all these audio channels available now, and no way to 
easily distinguish one from another. I have bluetooth hearing aids, and will 
attest to that ringer blasting into my ears when a call comes in. I have to 
keep my ringer volume set low and not have the buttons involved, to prevent the 
overwhelming sound. That means the volume coming from the phone speakers is 
lower, as well.
The only final answer would be that Apple would code all their audio channels  
so they route through a dedicated mixer either through the control center or 
give access to app developers. Being able to individually and independently 
control each audio channel is really the only complete answer to these problems.
Just counting on my fingers, I believe there are 4 output channels (phone 
headset, speakers, headphone jack, and Bluetooth) not all of which are stereo. 
Then the input sources as Tim wrote. A very complex traffic pattern to control, 
just as in a recording studio or live performance stage, where there is a sound 
engineer at the control board, deciding on what goes where.
