Hello,
I am not directly familiar with how QuickTIme and captioning works, but in 
general one creates a text file with time stamps and text, sometimes in XML but 
I believe there is another format also. 
If you are planing on exporting to youtube, then youtube can do a auto 
transcription which can be cleaned up. I have read through the process and 
discussed it some with co-workers, but I Have never tried it personally.
There are also several companies that will do transcription for you and charge 
a per minute rate.
Let me know if I should give you more information about any of these techniques.
