Hi, I've seen VoiceOver crash as well if you're typing extremely fast on the keyboard, switching back and forth. If you end up doing something just a bit too fast, it can crash too. It's really hard to describe and even harder to introduce.
As to your own problem, I've seen it on webpages. I've already given Apple a thorough explanation as to what is going on. You can, however, try hitting VO-Command-F8 to start VoiceOver Quick Start. That sometimes gets it back up and running.
