I believe this is a function of the firmware in a particular model of Bluetooth speaker. Beginning with iOS 13, certain brands of Bluetooth speakers began to not play VoiceOver through the speaker while playing music or other content. For example, I have a Bose Bluetooth speaker, that when playing music on this speaker, VoiceOver speech continues coming from my iPhone, while on my Jawbone Big Jambox speaker, both music and VoiceOver speech come through the speaker. I am not aware of a way to change this behavior on either device.
