Pressing either Fn + F12, or VO + the Equals/Plus key, increases the system volume. This is not the same as increasing the volume of VoiceOver, relative to the system volume.
To adjust the volume of VoiceOver, relative to the system volume, you need to access the Speech Attributes rotor. This is done using VO + CMD + Shift + left/right arrow keys, to select attribute categories, then using the up/down arrow keys to adjust attributes within a selected category. If adjusting the system volume does not resolve your issue, you may need to adjust the volume of VoiceOver relative to the system volume.
