Several things. First, go into the phone settings and under phone, look for "allow Calls on Other Devices” Make Double Tap and make sure the Mac is on. On the Mac, Go to Facetime and under the Settings, and make sure enable this account is still checked and Calls from iPhone is still checked. Finally, make sure your iPhone is signed into iCloud. If you are unsure, make that your first priority.
