I forgot to add that I've rarely encountered this issue on my 14-inch MacBook Pro (and those instances that I do experience are due to Web pages loading and other reasonable causes). If it were happening to me, I'd be lodging the support case.

Try visiting applevis.com, where you'll find thread upon thread of complaints about "not responding" (previously "busy") problems with VoiceOver for Mac, year after year, and with no resolution.

The best you can do is probably lodge a support case with Apple.
