Do all of the settings under VoiceOver -> Navigation seem correct? I’m not having any focus issues, so, without a clue as to what it might be, I can’t help any further. You may need to reset everything to defaults and reinstall applications. I would suggest taking this up with Apple support before making any drastic changes, though. They should be better at diagnosing the problem than anyone on this list.
