Ok, I've tried this atleast 20 times so I'll pass along this tip. In voiceover utility, go to web and uncheck the box that says "Move the VoiceOver cursor to it When loading a new webpage:". I also have the speak webpage and speak summary unchecked. Now, when you have that problem you can refresh the page and have no problems.
As I said, this has been very consistent for me, but now that I posted it, I wouldn't be surprised if it blows up in my face. Without that first box I mentioned unchecked, the page refresh doesn't seem to work.
