There is also an app called Accordio. It's a piano style accordion, but it isn't VO accessible. If you triple click, it will work nicely though. I have it, and I play the little accordion on my iPod Touch all the time. I'm also totally blind, and it doesn't present too much of a problem. If you hold the iPhone to with the home button facing your chest, the chord buttons will be on your left and the piano keys will be on the right. The chord buttons are a circle of fifths as you go from the end furthest away from you toward your chest. It's a lot of fun.
