Hello David, It sounds as though what you need is VisioVoice from AssistiveWare. I can't speak from personal experience, but my husband helps low-vision Mac users to set up their computers with VisioVoice and they really love its flexibility. It should give you the colour combinations that suit your sight.
E-mail me off-list and I'll get my husband to tell you all about it. The down side is that it costs quite a bit. A lot less than JFW, but it isn't what you'd call cheap. The up side is that it comes with the Infovox iVox voices of your choice, which means that you could have a Mac with a British accent.
