Are you using a 13 inch MacBook Pro? If yes, are you running Windows 7?
AAny thing that we can find that might give us a clue what is going on would
be very helpful.

I also have a 13 inch MBP running windows under VMWare Fusion with no
sound issues whatsoever. I'm running Jaws 12 over there as well. My MBP
is 15 months old.

VoiceOver, and is not setup like a standard Apple application. Instead
it has been ported over to Apple using its' own speech and essentually
the same keys as the Windows version. But, it is better than nothing.
