Hi,
I didn't experience this when I upgraded my iPod Touch.  DO you have any more 
information about how the crash happened?  Was there a dialog that popped up?  
Did you try cmd-f5 to restart Voiceover?  You sometimes need to hit it several 
times.  This seems strange...
