Michael:

There is a video button in the call window that lets you start/stop
video.  It took a bit of playing around to find it.  At least in my
case. VO wasn't automatically showing me all of the contents of my
window until i interacted with and manipulated one of the splitters.
Nowe, once I set it to reveal the whole window's contents, they are
reliably there.

I can be more help to you when i'm home again and in front of a mac.
My workplace is still stuck using windows, sadly.
