I have had this problem here as well. What I did was go into settings >
Sounds and sure you have an option set where you can control your sound
volume with the volume buttons on the phone; so every time I loose
sounds now I just give the volume buttons a couple of taps and sounds
are back once again.

Actually, I'm writing because my iPhone has been falling silent now, and I
either have to go plug it in to iTunes to reset voiceover, or flag down someone
sighted to go to settings. This again was confirmed by my 13-year-old geekette
daughter.
I've only noticed this fairly recently, but it is very much a concern since
this is my only source of phone contact and I don't always travel with my
computer available.
Has anyone mentioned this tendency to Apple Accessibility?
