I am not sure what folks mean when they say "crash".
I have noticed that since the 10.6.7 update, I seem to lose speech more often 
than before. However, VoiceOver is still running, just no speech. You can tell 
because when you do Command-Tab, you can hear the clicks as you switch 
applications. Except that no speech is forthcoming.
When this happens, you simply press Command-F5 to turn VoiceOver off, wait a 
few seconds, then turn it back on. Once VoiceOver is back on, everything is 
back to normal.
The other small issue I noticed is that whenever there is a "System Wide 
Server" message, VoiceOver has a hard time focusing on it to read the message. 
So if I don't catch it the first time it pops up, I lose it.
Other than that, everything is normal here.
