Hmm. I might have to run my external drive version of Lion a bit more. I'm 
still not ready to reinstall it oficially, but
from what I've seen, at least on my system, com.apple.doc.extra isn't going 
away. Yes, I know that all I have to do to get
rid of it is press command tab, but that just seems like a very unnecessary 
step, especially right at startup, and especially
since there was almost never any weirdness like that under SL. Anyway, I'll try 
running the version of Lion I have on an
external drive once in a while and see if com.apple.doc.extra ever does go away.

I have found that, after a few days, the comApple Extra thingie goes away even 
after restart.  Before that, I just pressed
command+tab to get rid of it. 

As for your not being able to open the Apple menu structure, very strange.  
That never gave me trouble.
