When you hold down the option key at start up, the drives appear on the screen. VoiceOver can't read it because the system is not running yet. Assuming your Mac is only running Lion, that drive will be the first one, your USB drive will be next, and the recovery partition will be last. So press right arrow once and then enter to boot from your USB drive.
