Hi,
I am having the same issue even with Lion. I've found that most of the time, if 
I press the keyboard shortcut for going to desktop VO-Shift-D, VoiceOver comes 
back speaking.
Give that a try.
Also, a lot of times, it's because I have Safari running. If I quit Safari, 
this problem doesn't happen as often.
