Figured it out, and hopefully this will help others.  

Apparently I had turned the speech volume down, I used ctrl-option-command
arrows to move right then up until I got some volume back and was able to
turn it the rest of the way back up from there.

Try a three finger double tap on the track pad with the track pad commander
on.
