I have no idea what happen in your case, but I updated two iPhones (4S), an 
iPad 2, and two first generation iPads with no issues. I updated my iPhone and 
my sighted wife's iPHone using VoiceOver and the iPad 2, and first gen iPad. 
THe second first gen iPad was done without VoiceOver. In every case the entire 
process went without a hitch.
So, I anticipate that most users will not have a problem.
