Hi, For me, it's a case of creating a new email, pressing Command-Shift-A to attach a file, VO-Cursoring over to the image list view, VO-Shift-downarrow to interact with it, moving to the downloads folder, selecting with VO-spacebar & browsing to the file in question & selecting, again, with VO-spacebar. Anyway, I think there are some limitations to hot key usage while in other subroutines.
