I finally encountered this. I cannot reliably reproduce the issue, but I have seen it. Not sure exactly what conditions if any need to exist for the problem to occur either. The one thing I have always experienced and never quite understood is why you get a phone call and if you unlock the screen, the Phone app is always sitting there in the foreground. That behavior has existed for a very long time and always something I wish APple would change.
