Hi Buddy,

I had similar issues for a while and it had something to do with my old 
MobileMe account as the iPhone was originally synced with MobileMe contacts.  
If I remember correctly, I first fixed up everything on the Mac like you did, 
then I turned off iCloud syncing on the phone, made sure that all Contacts on 
the iPhone were gone then deleted and re-added my iCloud account to the iPhone. 
 If your iCloud account used to be a MobileMe account, it doesn't appear to 
work properly until you delete then re-add it as an actual iCloud account.

Everything works perfectly for me now, I can add something on my iPhone, it 
syncs to the iPad and all my Macs just like it should.  This hopefully does 
something for you as well.
