A couple other points. 1. THere is an option to have the VoiceOVer cursor jump to where the keyboard cursor is. So for example on Google main page it would be in the search box.
2. Perhaps you are using an older Mac OS or not using webkit? In the older systems where I am not quite sure what is considered older, the entire screen would sometimes get updated when only a portion of the screen should have been updated. This would cause VO to jump to the top. ARIA live regions have helped resolve this at least with Lion and Webkit / Crome. Though don't run out and get the current (today) webkit, I have had it use a significant CPU load on Facebook, and had CBS.com web site constantly crash VoiceOver. No, I have not reported this.
