I have a USB audio interface which also serves as a control surface. I've
never seen this issue, and this thing interfaces via USB.

I noticed a similar problem upon installing Lion when it first came out. it
was a big problem for me every time I switched from headphones back to
regular speakers. Apple came out with an update that addressed the issue,
and, with the exception of a couple of times since, I have never had an
issue with this again. It has happened not at all yet under Mountain Lion.
