Sadly, the problem is not with you. It's a problem (bug, if you will), with 
VoiceOver and Mac OS X. It's been like that ever since I've started using the 
Mac in Lion. Other people have confirmed the problem with me, including Apple's 
accessibility department.

It's odd that it's such a problem on the Mac but not with the iPhone. It's a 
shame too as I'd love to have my Mac work with my bluetooth headset. And these 
days most wireless headsets are bluetooth; there are less and less that are 
dongle-based.

One solution may be for Apple to include a VO short cut key to revert VO to the 
built-in speakers which could be used if VO gets lost in bluetooth somewhere. 
However, I think the problem may be more serious than this as I, like you, 
typically have to restart my Mac once I lose VoiceOver in that way, even when I 
have sighted assistance to change the VoiceOver sound output.

If this situation ever changes I would certainly love to hear about it as I 
really miss not using bluetooth headset for VoiceOver.
