I suppose three things happen. First, Mail creates a smart mailbox which contains messages from your VIPs. As I understand it, these messages will still go to the mailbox they would ordinarily go to, but a copy is also kept in the VIPs mailbox. Second, this smart mailbox is added to your favourites bar. It can be removed. I have removed my VIPs mailbox from favourites. Third, you then have the option of choosing to only be notified of new mail from VIPs. This, as I understand it, does not necessarily mean you get no notification of other mail, but only that you can choose only to get notification centre notifications of mail from VIPs. This is the reason I have started using VIPs as I don't want to get notified of every email from all the different lists and newsletters I subscribe to, but it's useful to be notified of mail from real people, or at least the important ones. So, for example, I still get a little beep on my iPhone every time I receive an email, but only those from VIPs appear in the notification centre and are automatically read out by VoiceOver.
