Scott, The other issue could be that Voiceover simply lost focus with the mouse pointer. This has happened to me when attempting to make formatting changes by accessing the format bar or something like that. What I ended up needing to do was to route the mouse pointer to the VoiceOver curser, and then click. This fixed the issue. I do not remember precisely what caused this issue for me, but it was always on a file that I had created. If it is a file that you yourself have not created, then, as Anne said, the file could be locked, and duplicating it should permit editing.
