Hi, yes, there are improvements. Not many but, I believe some of them are extremely important. For me, the most important is the ability to access 3rd party icons in the status menu. This essentially gives voiceover users access to hundreds, maybe thousands of apps we otherwise couldn't use. Couple this with a more reliable drag and drop. Not perfect but, better. Improvements to the new voices that were added in Lion 10.7, etc. And I think There was enough improvements to voiceover to justify the $20.
