I know this may not be a popular suggestion as it would cost about 3 or 4 hundred dollars but I use DocuscanPlus from Sarotech and the Mac App is a free front-end to access the main application. It can of course, read stuff from a scanner and it can process PDF documents including those that were originally scanned. It works pretty well. You can then take the saved output and save it to your computer as a rich text file in case you need to edit the contents like in a form.
