Yes, this happens to me too, a lot.

I use column browser view in Finder. One time I notice it happening a lot is 
when , say, I have a folder called Downloads and inside that folder a file 
called Document A. I'll interact with the file browser and VoiceOver will 
announce I am on Downloads folder. I will move to the right with VO-Right Arrow 
so that the focus is on Document A but VoiceOver will read Downloads folder, 
Document A. I used to get confused by VoiceOver announcing Downloads folder, 
thinking it hadn't moved properly and so I would VO-Right Arrow again, which 
would have the unintended consequence of moving me into the information/details 
column instead of the file name column. 

I also notice what you describe happening to me in Mail. When I move to a new 
Mail message or conversation thread VoiceOver will often announce the previous 
message as well, which means I have to often double check what message I'm on 
because VoiceOver has told me two seemingly different things.
 
I'm just not sure whether this is a bug or feature of VoiceOver.
