How are you attempting to read character by character? In most applications with voiceover, unless you are in an application where you can edit, you have to interact with the text you want to read and use vo-shift and the arrows. In editable areas in applications that allow editing you can actually move character by character but Preview isn't allowing you to edit the pdf, just to read it.
Navigate to the place where you want to read the characters; you will already be interacted with the text to do this.
