I wonder if it doesn't depend on whether the video is being played by
Flash or HTML5. Flash is not accessible while HTML5 is. I have opted in
for HTML5 video when possible by going here:
http://www.youtube.com/html5
but there are still some videos that play with the Flash interface
because of some weird codec they use or advertising requirements.

A lot of ways the volume control will be located using the form controls, or by
going through the page with vo right. but other times its imply won't be there.
