It seems you have ath5k and MadWifi loaded at the same time. This probably
also explains the strange MAC revision "0xffffffff" - or your card is
broken. Last but not least, I'd expect this to be another case of "AR5007
card mis-recognized as AR5006 by lspci", see ticket #1192 for a solution.
